Encore Communities has experienced Social Service Professionals who are available to assist you with your needs throughout your stay in our facilities.  They will coordinate services with you, your family members, and various members of our Encore Communities health care team as needed. Some services offered by Encore Communities are:

• Assistance communicating with family and health care professionals.
• Scheduled care conferences including patient/resident, family and other staff care providers to establish and monitor progress.
• Support for patient/resident and family during times of grief, loss, change and crisis.
• Assistance with resolution of difficult decisions.
• Resident advocacy, assurance of rights.
• Intervention of social/emotional needs.
• Assistance in accessing community resources and finances.
• Comprehensive discharge planning, beginning on the date of admission at Northwoods Lodge. Progress is tracked throughout your stay to prepare for a smooth discharge experience.
•  Assistance in obtaining living arrangements and options after discharge, including independent living, assisted living, retirement centers, etc.
• Assistance in accessing home care services.
