Short-Term Skilled Nursing & Rehabilitation
We specialize in high-quality, 24-hour skilled nursing rehabilitation, providing round-the-clock medical care to patients recovering from an injury, hospital stay, stroke, or other physical conditions.
Conveniently located adjacent to our independent living and assisted living communities, we’re committed to helping you recover from a wide range of medical conditions and surgical procedures. Outpatient Therapy
The Northwoods Lodge team can provide outpatient therapy at a patient’s home or patients can come to Northwoods Lodge and get therapy in our state-of-the-art rehabilitation gym. No matter where we provide therapy, our goal is always to improve people’s health so they regain as much independence as possible.

Workers’ Compensation Program
Encore has a designated workers’ compensation team. We provide cost-effective, coordinated care and rehabilitation to help our patients achieve the best outcomes possible. Our team works with payers, claims adjusters, and nurse case managers with a collaborative approach to patient care and rehabilitation. Our team will develop a customized care plan, centered around the patient’s needs, communicate any updates or changes with the adjuster and case manager, and assist with discharge planning.
